Care Coordinator

A local community organization providing resources and support services to help older adults, individuals with disabilities, and their families maintain independence and quality of life. Services include connecting individuals with community resources, transportation, benefits assistance, and other supportive programs.

The Care Coordinator supports veterans in accessing personal care and supportive services that allow them to remain independent in their homes and communities. Responsibilities include assessing participants, developing and monitoring service plans and budgets, coordinating care and resources, educating veterans and providers, conducting community outreach, and maintaining program data and compliance.

Starting Wage: $21/hour
Schedule: Full-time

Essential Functions and Duties:
  • Conduct community outreach, marketing, and education to promote the program and referral process
  • Complete veteran intake, eligibility screening, assessments, enrollment, and reassessments
  • Develop and monitor individualized service plans, budgets, care needs, and service delivery
  • Educate veterans and caregivers on program requirements, resources, and employer responsibilities
  • Assist veterans with finding caregivers, service providers, community resources, and supportive services
  • Maintain case files, documentation, data entry, reporting, audits, and quality assurance requirements

Qualifications:
  • Human services or social work degree preferred but not required
  • Two years of related case management or service coordination experience preferred, or an equivalent combination of education and experience
  • Knowledge of case management, person-centered planning, service coordination, and budget development/monitoring
  • Understanding of veterans, older adults, individuals with disabilities, caregivers, public benefits, and community resources
  • Strong interviewing, assessment, problem-solving, advocacy, negotiation, and client support skills
  • Excellent verbal and written communication skills with the ability to build professional relationships
  • Strong organizational skills with the ability to manage multiple clients, meet deadlines, and maintain accurate documentation
  • Proficiency with Microsoft Office and databases, sound independent judgment, and the ability to work effectively in a fast-paced team environment
